Print the pages below.  It is recommended that page one be printed on a heavy card stock type paper.  The other pages can be printed on regular
 copy paper.  Cut around the pages and trace the letters.  Put the pages  in order and place them on the dotted box
 on page one.  Staple in place.  Read!!
