Reading for a Gold-Reading Incentive Sheets

These are available in an easier version as shown or and harder
version with 15 blanks.
To encourage your youngsters to read, print an incentive sheet for
 them in the version of your choice.  Give them a designated amount of them time to read books at home.  As they read, have them write the title
of the book on the line.  When the time period is over, have the children
see how many books they have read and award them
the appropriate medal.
